It seems like the more time goes on the more we come to understand it but without any "major" breakthrough's. When I saw the doctor last week, he said that at 60, 4 or 4.5 was OK and the number goes up as you get older, presumably because prostate cancer is slow-growing and you are more likely to succumb to something else in the meantime. I suppose that your worries, John, are down to the rate of increase and whether it's popped up elsewhere and, if so, where. All I can do is wish you the best and hope the shadow on the humerus is from the knock you mentioned. Can't they do a bone scan where you have an injection of some radioactive jollup and then drink buckets of water to flush it out of your system before being scanned to see if any bony bits have taken up more radiation than it should have? I'd had a standard bone scan back when I was diagnosed (with a PSA of 22+) and that showed nothing but the usual arthritic knees.... This one had a radioactive injection that came in a lead covered syringe in a lead box, followed by a scan. I did ask if we could consider the original bone scan as a "base line" indication, but I was told that the two scans are so different that the results aren't directly comparable. Anyway, as you say, my problem is the rate of change of the gradient of the PSA v. Time graph...not the pure value of the PSA. The gradient is getting steeper much too quickly for comfort... They do say that the right hormones and drugs at the right time could give me 20 years, 10 is more likely, and 5 should be assured. Do I think I'll last to 88? 78? 73? Never thought I'd get to 40......! molemot Posted September 2, 2014 Report Share Posted September 2, 2014 Yesterday was the latest consultation at the Marsden. I had plotted all the PSA results from day 1 onwards on a graph...on the laptop....Anyhoo, the graph was very instructive. It depicts the PSA hurtling downwards at the start of treatment, getting lower and lower as the treatment progressed through the hormone injections and bicalutamide pills and the radiotherapy and the trans urethral resection of the prostate......and then dribbling along at a low level after treatment ceased some 4 years ago. Then it starts to go up...slowly at first, but back in the January consultation it had started to accelerate...the gradient of the line was getting steeper, wot means it was going up faster and faster. The January result was 1.8 and by February this had become 2.2.......0.4 in one month. Much sucking of collective teeth. Had a scan...which seemed to show it was all still inside the prostate, apart from a shadow on the right humerus...the upper arm bone. This might have been due to my accident when I ripped the shoulder apart in 2000....but at this point I got a touch of the Eyores and feared the worst. Thus, when this consutation was due, I went to my graph and looked at the gradient and waved my magic ruler over the trace and predicted that the PSA would be in the low teens....13 or 14,say...which would fit perfectly with the trend on the graph. Such was my prediction....it appears that the medics had done something similar, but they were expecting it to be 5 or 6. Well, they know more than we do...... The actual figure turned out to be 2.7!!! It had gone up, but by only 0.5.......so the consultation was quite cheery. What seems to be the defining progression is how long it takes for the PSA to double..and that, in my case, has been about a year. Thus the decision we reached was 1) no surgery....even though there is a chance of a total cure, it is only about 30% or so with a 70% chance of spending the rest of my days dribbling into nappies and still having the tumour and all the associated ruin....don't fancy that; life is about the quality of it and not simply breathing in and out. Then there was 2)... no treatment needed at present....the PSA trend had become much less pronounced and it was better to wait until it has gone up to around the 13 point or I start to get symptoms before we go back on the hormones...as they lose their effect in the end, and anything that will delay that point is beneficial. SO....that's my state at the moment!!! Much better news than I had anticipated. I had my 6 monthly checkup last week after my annual checkup was shortened due to swelling and irritation in the affected area. He said everything was ok, not perfect, but ok and that there is currently no sign of further issues. I'm still on 6 monthly checks though. I'm at the Marsden for the usual 6 month howgozit in a couple of weeks....frankly, not too worried about the cancer at the moment, priority is getting the left arm working properly again. Dull bugger....fancy tripping over the radio controlled tank....! Shoulder dislocations are a tedious business ... axillary nerve is not working...just like when I did the right one in 2000....but that all came back well enough, so shoud be OK after 6 months or so. I'll report back on the Marsden prognostications when I know them!!
